Living Donor Selection Criteria
Eligibility Criteria
- Excellent overall health
- Age 18 or older
- BMI less than or equal to 35
- Contraindications to Living Kidney Donation
Absolute Contraindications
- Medical
- Significant coronary artery disease
- Symptomatic valvular disease without correction
- Significant peripheral arterial disease or carotid artery stenosis.
- History of hypertension and age < 50 years old
- Moderate or severe chronic lung disease
- Creatinine clearance < 80 mL/min/1.73m2
- Diabetes mellitus
- Microalbuminuria > 100mg/24hr
- History of chronic kidney disease (e.g., glomerulonephritis, nephrocalcinosis)
- Sickle cell disease
- Active sarcoidosis
- Infection
- HIV infection
- Active hepatitis B or hepatitis C infection
- Malignancy
- History of melanoma
- Financial/social
- Severe psychiatric illness
- Alcohol abuse, or recent use (6 months) of cocaine, amphetamines, non-prescription opiates, any IV drugs, bath salts
- Incarceration or home arrest
Relative Contraindications
- Medical
- Hypertension
- Active use of medications that may cause kidney dysfunction
- History of thrombosis or embolism
- Urologic abnormalities of donor kidney
- Nephrolithiasis (recurrent)
- Impaired glucose tolerance
- Autoimmune disease
- Sickle cell trait
- Creatinine clearance 80-90 mL/min/1.73m²
- Microalbuminuria 30-100mg/24hr
- Malignancy
- History of non-cutaneous malignancy
- Financial/social
- Refusal to receive blood products
- Currently serving parole or on probation.
Disease specific notes
Polycystic kidney disease: Refer to UMMC policy on PKD in potential donors
Nephrolithiasis: Potential donors with a history of kidney stones will undergo complete metabolic testing with serum iPTH testing and 24 hour urine stone risk assessment via Litholink. Determination of donor acceptance will be based on a donor-specific assessment of current stone burden and risk of stone recurrence.
